ETL Tool goes blank screen after selecting driver jar

When I select JDBC Driver to connect to MS SQL Server, the window goes blank.

• neo4j version - desktop version 1.1.13, etl-tool version - 1.3.1
• which database do you want to import from - Microsoft SQL Server

Any help would be great since we are having a hard time extracting data from SQL Server, converting the data into csv and then importing the csv into Neo4j.
